Transition into schools and settings

- Transition is a continuous process of sharing ongoing information into schools and settings. On referral into the Centre, a child is given a key worker who will :
- Make contact with a nursery
- Invite nursery, parent and care staff to the child’s multi disciplinary review
- Attend meetings about the child in all settings when invited
- Share information and agree goals for the Individual Education Plan
- Suggest any training to further enable and develop the setting
- Relay information to the Special Needs Advisor for the school they are due to enter school for their reception year
- Consider how their needs are best met in an educational environment
- Organise a final transition meeting in school with the SENCO entry to update current information on Individual Education Plan, Teaching and Learning Programmes and Strategies, Assessments, Ongoing Therapy Support and Tracker.
